Full-Time Palliative Care Coordinator

In-Office Position | Pasadena, CA

Help coordinate compassionate care that truly makes a difference.

We are seeking an organized, compassionate, and detail-oriented Palliative Care Coordinator to join our growing team. This position plays a key role in coordinating patient care, scheduling services, processing referrals, obtaining authorizations, and supporting our interdisciplinary care team.

Schedule: Monday - Friday | 9:00 AM - 6:00 PM

$21.00 - $25.00/hour (DOE)

Spanish speaking preferred

What We're Looking For

  • Strong computer and customer service skills
  • Excellent communication and organizational abilities
  • Hospice, home health, or palliative care experience preferred
  • Ability to multitask in a fast-paced healthcare environment
  • Professional, compassionate, and team-oriented attitude
  • Valid driver's license and reliable transportation

Key Responsibilities

  • Coordinate palliative care referrals and admissions
  • Obtain insurance authorizations and re-authorizations
  • Schedule patient visits and coordinate care services
  • Communicate with physicians, nurses, social workers, and care teams
  • Review documentation for accuracy and timely submission
  • Support patient and family needs while ensuring quality care delivery
  • Maintain accurate patient records and reports
