    JOB SUMMARY

    The Palliative Care Coordinator is an integral part of the team assisting providers to provide excellent care, setting the standard in our community.

    DUTIES & RESPONSIBILITIES
    • Receive, process and coordinate referrals and following up with patient in a timely manner to coordinate admission
    • Scanning and upload patient documents
    • Assure orders are sent to physicians in a timely manner
    • Handling incoming calls and other communications
    • Maintain organization of the office common area
    • Obtains and documents prior authorization for services from insurance providers
    • Schedule providers to see patients on a recurring basis
    • Assists to maintain liaison relationships with hospitals, facilities, insurance personnel, providing information and education on organization services, coverage issues and related areas.
    • Resolve appointment conflicts for staff
    • Produce and process schedules and reports as they are requested by administrative staff
    • Assure admit packets are stocked and ready for both admissions
    • Assist with claim denials

    JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
    • At least 18 years of age
    • Ability to read and follow written instructions and document care given
    • Satisfactory references from previous employers
    • Is self-directing with the ability to work with little direct supervision
    • Good oral and written communications
    • Good organizational skills
