Responsibilities:
Conduct home visits for patients requiring pain management
Perform comprehensive pain assessments and develop individualized care plans
Prescribe appropriate therapies including medications, injections, and non-pharmacological approaches
Monitor treatment effectiveness and adjust care plans accordingly
Educate patients and caregivers on pain management strategies and safe medication use
Maintain clear documentation in the EMR system
Collaborate with physicians, specialists, and interdisciplinary teams
Requirements:
Licensed Nurse Practitioner in the state of Nevada
DEA license with Schedule II-V prescribing authority
Experience in pain management, geriatrics, or palliative care preferred
Strong clinical judgment, compassion, and communication skills
Reliable transportation and willingness to travel to patient homes
