The Transportation Logistics Coordinator is responsible for coordinating and optimizing all transportation logistics for PACE KC participants, ensuring safe, reliable, and participant-centered service delivery. This role functions as a core member of the PACE KC Interdisciplinary Team (IDT), integrating transportation planning into participant care plans and daily operations.

This position does not provide direct supervision of drivers, but is accountable for end-to-end transportation planning, routing, dispatch coordination, and communication. The Coordinator must maintain operational continuity by filling in as a PACE KC driver when needed, ensuring uninterrupted service delivery.

Transportation Logistics & Route Optimization

  • Develop, manage, and continuously optimize participant transportation schedules and routes using dispatch systems.
  • Coordinate daily dispatch operations, ensuring alignment with participant appointments, center attendance, and care needs.
  • Monitor real-time transportation activities and adjust schedules to address delays, cancellations, or urgent care needs.

Interdisciplinary Care Integration (IDT)

  • Serve as the transportation liaison to the PACE KC care team, participating in:
    • IDT meetings
    • Care plan discussions
    • Family meetings and case conferences
    • Ensure transportation plans are integrated into each participant’s individualized care plan.

  Participant-Centered Coordination

  • Coordinate transportation services that reflect each participant’s medical condition, mobility status, and personal preferences.
  • Communicate proactively with participants, caregivers, and families regarding schedules, delays, and changes.
  • Support door-to-door service coordination and safe transfer planning for participants with mobility challenges.

Dispatch & Operational Management

  • Act as the primary coordinator and lead for daily dispatch operations, working closely with drivers and transportation leadership.
  • Work closely with the PACE KC Center Director or their designee to ensure optimal coordination of participant needs and issue resolution.
  • Maintain accurate scheduling, route data, and transportation documentation in dispatch systems.
  • Identify opportunities to improve efficiency, reduce drive time, and enhance participant satisfaction.

Compliance & Safety Coordination

  • Ensure all transportation logistics adhere to:
    • Department of Transportation (DOT) regulations
    • Centers for Medicare & Medicaid Services (CMS) requirements
    • ADA transportation standards
  • Support implementation and adherence to transportation safety protocols and procedures.

Incident Coordination & Documentation

  • Coordinate reporting and documentation for transportation-related incidents, delays, or service issues.
  • Partner with the Transportation Manager to support incident investigations and quality improvement initiatives.

Data, Reporting & Performance Monitoring

  • Track key transportation performance indicators (e.g., on-time pick-up/drop-off, route efficiency, service utilization).
  • Maintain accurate records and generate reports to support operational decision-making and regulatory compliance.

Cross-Coverage & Driving Support

  • Serve as a backup PACE driver as required to maintain uninterrupted transportation services.
  • Maintain all required licensure and certifications to safely operate PACE vehicles.

Continuous Improvement & Operational Support

  • Identify process improvement opportunities related to routing, scheduling, and participant experience.
  • Participate in quality improvement initiatives, training sessions, and operational planning efforts.

Educational Requirements

  • High School Diploma or GED required; Associate’s or Bachelor’s degree preferred.

 Experience & Qualifications

  • Minimum 3–5 years of experience in transportation logistics, dispatch, or operations (healthcare or eldercare preferred).
  • Strong working knowledge of:
    • Transportation routing, dispatch systems and Microsoft Office (Word, Excel)
    • DOT, CMS, and ADA transportation regulations
  • Valid driver’s license with a clean driving record and ability to meet DOT certification requirements.
  • Familiarity with Kansas City metro geography and traffic patterns.
  • Proficiency in Microsoft Office and transportation/logistics software. Core Competencies.

  • Participant satisfaction scores related to transportation
  • Reduction in missed appointments due to transportation barriers
  • Compliance audit results (DOT/CMS/ADA)
  • Incident response and resolution timeliness

 Physical & Work Requirements

  • Must be able to drive PACE vehicles and assist with participant mobility as needed.
  • Ability to sit, stand, walk, and lift weights consistent with transportation support duties.
  • Flexibility to work varied hours, including early mornings, weekends, and holidays as needed.
