    Summary:

    The primary goal of this position is to increase enrollment of potential participants to the Living Independently for Elders (LIFE) program of Catholic Health(CH) while incorporating the philosophy and mission of CH. Responsible for fostering relationships with referral sources including hospitals, physicians and various community organizations and key decision-makers to gain market share for LIFE. This position will focus on developing and maintaining external relationships with all potential referral sources.

    This position will be responsible for receiving pre-enrollment information from individuals who express an interest in the LIFE program. This includes providing an explanation of LIFE, eligibility requirements, philosophy and mission of the program as well as the services provided. This position will also be responsible for understanding and applying all Federal, State and local regulations and procedures regarding marketing, intake and enrollment of PACE participants. A working knowledge of MLTC's, Maximus, Medicaid, Medicare and utilization of Pooled Trusts will be integral to the position. Works collaboratively with Community Liaison to build relationships with community referral sources and other key stakeholders especially in the Spanish speaking community. Must possess strong interpersonal communication and customer service skills related to a geriatric population. Must be comfortable with public speaking presentations. Flexible scheduling of time is essential and may require some evenings and weekends as needed. Willingness and ability to travel between sites and throughout PACE service area is required. Spanish fluency both written and oral.

    Responsibilities:

    EDUCATION

    • Associates degree in business, communications, or health related field required
    • Bachelor's degree preferred


    EXPERIENCE
    • Two (2) years of customer service or sales experience required
    • Two (2) years of healthcare/social services/human services preferred
    • One (1) year of experience working with frail elderly population preferred
    • One (1) year of working with multi-cultural , multi-ethnic and hard to reach communities preferred

    KNOWLEDGE, SKILL AND ABILITY

    • Fluency in Spanish required
    • Must possess a valid driver's license and have a car available for work use
    • Working relationship/knowledge of senior health issues and the Program's service area
    • Strong computer skills required including word processing, spreadsheets, presentations, and data base management
    • Willingness to learn new software applications
    • Strong written, verbal and interpersonal communication skills
    • Ability to work independently
    • Ability to develop and maintain relationships and to work productively with all levels of personnel including clinical staff, families and caregivers
    • Accountable/Results oriented
    • Detail oriented
    • Experience in or willingness to learn Medicaid and Pooled Trust processes
    • Integrity and respect for others
    • Possess active listening skills, analytical abilities and teaching abilities

    WORKING CONDITIONS:
    • Normal heat, light and safe working conditions typical of most office jobs
    • Travel between locations and prospective participant's homes will be required
    • Travel to community outreach events will be required
    • Physically able to transport and set-up displays, presentation materials and supplies; for both indoor and outdoor events
