Part-Time Outpatient Therapist


The Therapist at Mentor Behavioral Health provides therapeutic interventions to youth, families and adults. Services are provided in homes, communities, office and virtually – whichever best meets the needs of individuals served.

Responsibilities:


+ Conduct diagnostic evaluations.

+ Provide therapy, family support, and crisis interventions to an individual, family and/or group using various treatment modalities.

+ Provide mental health consultation to stakeholders and treatment team members.

+ Develop individualized treatment plans in coordination with an interdisciplinary team

+ Develop and coordinate discharge planning.

+ Maintain timely and accurate health record documentation.

+ Attend regular staff meetings, clinical meetings and one-on-one sessions with direct supervisor to process current status of cases.

+ Provide 25 service delivery hours per week.


_Qualifications:_


+ Bachelor's, LSW + at least 3 years’ experience

+ Master’s Degree in human services + LPC, MFT
