    Description

    A healthcare facility is seeking an Outpatient Physical Therapist to provide patient-centered rehabilitation services within an outpatient setting. This position supports individuals through evaluation, treatment planning, and therapeutic interventions designed to improve mobility, function, and quality of life.

    Responsibilities

    Evaluate patients and develop individualized treatment plans based on clinical needs and functional goals.

    Provide outpatient physical therapy services using evidence-based treatment techniques.

    Perform therapeutic exercises, interventions, and education to support patient recovery and independence.

    Monitor patient progress and adjust treatment plans as needed.

    Document evaluations, treatments, progress notes, and other required clinical information accurately.

    Collaborate with physicians, healthcare providers, and rehabilitation team members to coordinate patient care.

    Educate patients and families on treatment plans, home exercise programs, and injury prevention strategies.

    Maintain compliance with clinical standards, safety guidelines, and regulatory requirements.

    Requirements

    Bachelor's degree in a clinical discipline (e.g., RN, PT, OT, SLP, or SW) or healthcare management required; Master's degree preferred.

    Minimum of 3 years of experience in rehabilitation leadership or administration.

    Valid clinical license as applicable by state.
