Outpatient Physical Therapist (PT) – Logansport/Peru, IN

Our client is seeking an Outpatient Physical Therapist (PT) for a full-time, direct-hire opportunity. This position will provide outpatient physical therapy services with flexibility between Logansport Hospital Therapy Services and Peru Therapy Services.

This is a 40-hour-per-week, day-shift position, Monday through Friday, with weekend coverage approximately once per month.

The Physical Therapist provides professional patient care and practices physical therapy by evaluating, planning, implementing, and documenting treatment.  The PT evaluates and treats patients in accordance with physician orders and develops individualized treatment plans designed to maximize each patient's functional potential.

Sign-on Bonus: Up to $15,000

May be eligible for relocation bonus

 

Physical Therapist Job Responsibilities

·        Evaluating patients and developing individualized treatment plans based on patient needs and achievable functional goals.

·        Providing physical therapy treatment through a variety of therapeutic procedures and assistive devices.

·        Revising treatment plans as needed throughout the treatment process.

·        Instructing and collaborating with Physical Therapist Assistants (PTAs) regarding treatment procedures and providing appropriate supervision.

·        Educating patients, families, and caregivers regarding exercises and therapeutic home programs to continue following discharge.

·        Monitoring patient progress and working toward improved functional outcomes.

·        Completing appropriate clinical documentation and billing in a timely manner.

·        Providing care in accordance with physician orders, hospital and rehabilitation department standards, the Indiana State Practice Act, the American Physical Therapy Association, and The Joint Commission.

·        Working at other locations as assigned.

·        If assigned to drive a company vehicle, maintaining a valid driver's license. If driving a personal vehicle, maintaining a valid driver's license and proof of insurance.

 

Physical Therapist Requirements/Knowledge & Skills

·        Graduate of an accredited school of Physical Therapy.

·        Minimum of a Bachelor's degree in Physical Therapy if graduated prior to 1/1/2016.

·        Doctorate of Physical Therapy (DPT) required if graduated after 1/1/2016.

·        International candidates must be graduates of an accredited school and possess a minimum of a Bachelor's degree in Physical Therapy.

·        Doctorate of Physical Therapy degree preferred.

·        Must hold or be eligible to obtain a Physical Therapist license in the state where services will be provided.

·        State licensure must be obtained within the first 90 days of employment.

·        Must be CPR certified or become certified within the first 90 days of employment.

·        Must maintain all required licenses and certifications according to expiration dates.

·        American Physical Therapy Association membership preferred.

·        Previous work experience is preferred but not required beyond clinical experience obtained through an educational training program.

·        Must be able to lift 50 pounds, assist with 100 pounds, and may be required to push up to 300 pounds.

·        Must be willing to work at other locations as assigned.
