    Position Title: Outpatient Coordinator
    Position Summary:
     Responsible for preparing admission documents, obtain copies of Identification cards and insurance cards, assisting patients with admission paperwork. Responsible for communicating any updates or patient concerns with pharmacies, hospitals, families, and facilities. Coordinates all transportation to and from groups and appointments. Helps transport patients when needed.  Keeps tracks of patients due dates for injections. Helps clients connect to additional resources if necessary.  Responsible for auditing charts, collecting signatures, and  updating admission orders as needed. Sends copies of psychiatric orders, psychiatric progress notes to appropriate providers after received. Assist with patient care as needed. Update physician and therapist with any patient concerns. Obtain updated medication list from hospitals, pharmacies, facilities. Assist providers coordinating follow up visits. Assist therapist with master treatment plans, reviews, and discharge summaries. Assist with providing therapist breaks by monitoring patients and serving lunch and beverages.

    Qualifications
    Education:

    High school degree or degree from an accredited college or university in social work, recreational therapy, or appropriate therapeutic field preferred. Combination of education and relevant work experience may be required.
    Experience:  

    Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, and/or psychiatric patients preferred.
    License: Will require valid driver’s license and a favorable Motor Vehicle Report (MVR), along with proof of insurance when using personal or hospital owned vehicle.
    Additional Requirements:  CPR & First Aide  certification and Handle with Care Training within 30 days of employment. Completion of additional  job specific training within 30 days of employment as required. All training and certifications must be maintained in good standing for the duration of employment. Must be at least 21 years of age.  May be required to work flexible hours and overtime including weekends, evenings, and holidays.
