    How you'll contribute

    An Outcomes Manager who excels in this role:

    • Coordinates collection, documentation, and submission of Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI) and Quality Indicators (QIs) data in accordance with CMS requirements and facility policies.
    • Partners with the interdisciplinary team to ensure timely, accurate data capture and supports education on QI documentation standards.
    • Works closely with nursing, therapy, case management, and medical staff to support accurate QI scoring and IRF-PAI completion.
    • Adheres to CMS regulations, payer guidelines, and organizational policies related to data integrity and timeliness.
    • May assist with audits, performance improvement activities, and data validation.

    What we're looking for

    • Education/Licensure: Clinical degree with current license to practice as an OT, PT, RN, SLP, or MSW.

    • Knowledge: QI competency; working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.

    • Skills: Basic computer and data entry proficiency; strong clinical assessment/analysis; effective verbal and written communication; excellent interpersonal skills; ability to collaborate across disciplines and meet strict deadlines with high accuracy.
