OPWDD Job Coach

The Job Coach is responsible for but not limited to the following: - Provide on-site job coaching for designated caseload - Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies DDPS and other required information. The Job Coach specializes in assisting participants with learning and gaining job skills.

The Job Coach specializes in assisting participants with learning and gaining job skills. The Job Coach provides one-on-one job training tailored to the needs of the participant. The Job Coach is responsible for but not limited to the following: - Provide on-site job coaching for designated caseload - Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies DDPS and other required information. - Act as a liaison between the person supported, family and employer. - Attend all staff meetings, Life Plans, conferences, and annual training requirements. - Ability to demonstrate patience, creative thinking, and exhibit great teamwork. - Provide overall assistance as assigned. Skills: - Strong communications both written and verbal - Detail oriented, multitasker. - Excellent time management skills - Strong computer skills
