About the Role

This isn't a role where you'll simply step into an established program. You'll help build what comes next.

As an Advanced Practice Provider on Chorus Health's Advanced Therapy Remote Patient Monitoring team, you'll play a critical role in launching and scaling a new model of care designed to help patients safely receive complex therapies while remaining connected to expert clinical support.

You'll serve as the clinical backbone of our remote monitoring program and the escalation point for the RNs monitoring patients receiving bispecific and CAR-T therapies. Your clinical judgment will directly influence how we identify complications early, respond quickly, support patients and caregivers, and work alongside treating physicians to keep patients safe throughout treatment.

You'll collaborate closely with physicians at our partner sites and with Chorus Health's Chief Medical Advisor to continually strengthen our clinical protocols as the program grows. You'll also have something clinicians rarely get the opportunity to do: help shape the technology and clinical workflows being built around the care you deliver, partnering directly with our product and operations teams.

Our clinical model has already demonstrated what's possible. Across 27 outpatient clinics, we've supported bispecific step-up dosing with a sub-2% admission rate. Now we're building the team that will help take that model further.

This is an in-person role based in New York City.

Compensation includes a $180,000–$210,000 salary, competitive equity, and a comprehensive benefits package including healthcare coverage, flexible time off, and a wellness budget.

What You'll Do

• Serve as the clinical escalation point for our RN monitoring team, providing advanced assessment and management for patients on bispecific and CAR-T therapies via remote (phone and video) monitoring at protocol-defined intervals.
• Grade and manage CRS and ICANS in coordination with treating physicians, initiating and adjusting interventions like dexamethasone or tocilizumab per protocol and within your scope of practice.
• Triage escalations from the RN team, determining urgency and disposition and directing next steps with our partner clinics and their on-call teams. 
• Coordinate treatment starts and dosing timelines with partner-site physicians so monitoring plans stay aligned across our sites of care.
• Document every encounter accurately and promptly in the system of record.
• Educate patients and caregivers on disease processes, treatment side effects, warning signs, and self-management.
• Mentor the RN monitoring team and help refine our protocols as we learn from every patient we monitor.

What You'll Bring

• A current NP or PA license, unrestricted in New York State, with a master's degree from an accredited NP program or graduation from an accredited PA program.
• National certification (ANCC/AANP for NPs, NCCPA for PAs) and current DEA registration.
• 3+ years of oncology or hematology experience, with the clinical judgment to make independent decisions in a remote monitoring setting. 
• Current BLS certification, plus the willingness to obtain additional state licensure as our partner network grows.
• Sharp clinical communication skills — you can accurately assess a patient's condition over the phone or video and know exactly when something doesn't add up.

Bonus points if you have:

• Direct CAR-T, bispecific, or gene therapy experience.
• Demonstrated experience assessing and managing CRS/ICANS.
• ACLS certification.
• Experience mentoring or leading clinical teams, especially in a remote or hybrid care setting.
