We are looking for an experienced registered nurse who initiates and coordinates the hospice care plan, supervises CHHAs and LVNs and provides skilled nursing hospice and palliative care services to our patients at their place of residence. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers.
