Are you passionate about helping people access the care and services they need to live safely and with dignity? Join the STEP Office of Aging as a Long-Term Care Assessor I and play a critical role in connecting individuals and families with long-term care resources and community supports.

In this position, you will conduct comprehensive assessments, determine eligibility for long-term care programs, and help individuals navigate available services. Every assessment you complete has the potential to improve someone's quality of life and ensure they receive the appropriate level of care.

What You'll Do

  • Conduct in-person assessments in hospitals, nursing facilities, personal care homes, and private residences throughout Lycoming and Clinton Counties.
  • Complete Functional Eligibility Determinations (FED) for nursing facility services, Community Health Choices (CHC), Albright LIFE, and other long-term care programs.
  • Review medical records, physician documentation, and supporting assessments.
  • Interview clients, family members, and other support systems to gather comprehensive information.
  • Assess physical, functional, psychosocial, and environmental needs.
  • Educate individuals and families about available care options and community resources.
  • Coordinate referrals to service providers and enrollment resources.
  • Maintain accurate electronic documentation and reports.
  • Participate in hearings, case consultations, and professional development activities.
  • Support office operations and community outreach efforts as needed.

Requirements

  • Bachelor's degree in social work or a related field.
  • One year of experience conducting functional, clinical, psychosocial, or eligibility assessments.
  • Ability to obtain Functional Eligibility Determination (FED) and Pennsylvania Individualized Assessment (PIA) competencies during the introductory employment period.
  • Valid driver's license, reliable transportation, and ability to travel throughout the service area.
  • Proficiency with Microsoft Office and electronic documentation systems.
