Fountain Care at Sunset Hills is seeking a compassionate and dedicated Occupational Therapist (OT) to join our growing in-house rehabilitation team. We are hiring for both Full-Time and Per Diem positions. This is an excellent opportunity to make a meaningful difference in the lives of our residents while working alongside an experienced interdisciplinary team committed to providing exceptional patient care.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled therapeutic interventions to improve residents' independence and quality of life.
• Assess functional abilities related to activities of daily living (ADLs), mobility, cognition, and safety.
• Collaborate with physicians, nursing staff, therapy team members, and families to ensure coordinated care.
• Maintain accurate, timely documentation in accordance with state and federal regulations.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate residents, caregivers, and staff on therapy goals, adaptive equipment, and home safety recommendations.
• Ensure compliance with all regulatory and facility standards.

Qualifications

• Current Missouri Occupational Therapist (OT) license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Skilled nursing facility or rehabilitation experience preferred, but new graduates are encouraged to apply.
• Strong communication, organizational, and clinical skills.
• Ability to work collaboratively in a multidisciplinary environment.
• Passion for providing compassionate, patient-centered care.
