Position Summary

Matrix Home Care is seeking experienced and dependable Occupational Therapists and Occupational Therapy Assistants for PRN home health visits throughout Fort Myers, Cape Coral, and South Punta Gorda.

Clinicians will provide therapy services to patients in their homes. Occupational Therapists will complete evaluations, develop individualized plans of care, and monitor patient progress. Occupational Therapy Assistants will provide follow-up treatment under the supervision of the Occupational Therapist.

Occupational Therapist Responsibilities

• Complete initial evaluations and reassessments
  
• Develop individualized plans of care and measurable therapy goals
  
• Assess functional abilities, home safety, mobility, and activities of daily living
  
• Provide therapeutic interventions to improve independence and quality of life
  
• Recommend adaptive equipment and home modifications
  
• Educate patients, caregivers, and family members
  
• Supervise Occupational Therapy Assistants as required
  
• Complete accurate and timely clinical documentation
  
• Communicate changes in condition to physicians and the interdisciplinary team
  

Occupational Therapy Assistant Responsibilities

• Provide treatment visits according to the Occupational Therapist’s plan of care
  
• Assist patients with activities of daily living, strengthening, coordination, and functional tasks
  
• Reinforce home exercise programs and safety instructions
  
• Educate patients and caregivers on therapy techniques
  
• Monitor and document patient progress
  
• Report changes in condition or progress to the supervising Occupational Therapist
  
• Complete visit documentation accurately and on time
  

Requirements

Occupational Therapist:

• Current Florida Occupational Therapist license
  
• Graduate of an accredited Occupational Therapy program
  
• Home health experience preferred
  
• Experience completing evaluations and plans of care
  
• Current CPR certification
  

Occupational Therapy Assistant:

• Current Florida Occupational Therapy Assistant license
  
• Graduate of an accredited Occupational Therapy Assistant program
  
• Home health experience preferred
  
• Current CPR certification
  

Background Screening Requirement

Under Florida law, this position is subject to background screening pursuant to Chapter 435, Florida Statutes. This includes fingerprinting and a review of eligibility before employment.
