Key Responsibilities
Complete ongoing interdisciplinary assessments and develop individualized care plans
Provides services that are ordered by the physician as indicated in the plan of care.
- Coordinate care with interdisciplinary team members
- Educate patients and caregivers on disease processes, medications, and safety
- Maintain accurate, timely documentation
- Communicate effectively with physicians, families, and internal team members
Develops treatment program and establishes goals for improved function.
Participate in part‑time scheduling as needed by the team
