    Position Overview

    Key Responsibilities
    • Complete ongoing interdisciplinary assessments and develop individualized care plans
    • Provides services that are ordered by the physician as indicated in the plan of care. 
    • Coordinate care with interdisciplinary team members
    • Educate patients and caregivers on disease processes, medications, and safety
    • Maintain accurate, timely documentation
    • Communicate effectively with physicians, families, and internal team members
    • Develops treatment program and establishes goals for improved function.
    • Participate in part‑time scheduling as needed by the team

    Qualifications
    OT Requirements
    • Current Washington OT license (or ability to obtain)
    • One year of occupational therapy experience preferred; home‑health experience a plus
    • Strong assessment, communication, and critical‑thinking skills
    • Ability to work independently in a field‑based role
    • Reliable transportation and valid driver’s license
