    Occupational Therapist (OT) - Home HealthSummitWest Care | Mesa County, CO Full-Time | Flexible Daytime Schedule$48-55/hour DOE + Mileage ReimbursementHelp Patients Maintain Independence at HomeSummitWest Care is a local, nonprofit home health agency seeking a compassionate, skilled Occupational Therapist (OT) to provide one-on-one care to patients throughout Mesa County.This position offers the opportunity to build meaningful relationships while helping individuals safely navigate daily life in the comfort of their own homes. You'll work independently in the field with the full support of a collaborative, interdisciplinary team committed to delivering high-quality patient-centered care.As a home health OT, you'll have the autonomy to focus on clinical outcomes, patient education, and individualized treatment plans without the pressure of unrealistic productivity expectations.Responsibilities:Perform occupational therapy evaluations, reassessments, recertifications, and dischargesDevelop and implement individualized treatment plans focused on safety, function, and independenceAssess patients' ability to performs ADLsConduct home safety evaluations and recommend adaptive equipment or environmental modificationsEducate patients and caregivers on home programs, mobility strategies, and safety techniquesMonitor patient progress and adjust treatment plans as appropriateCollaborate with nurses, therapists, caregivers, physicians, and familySupervise and support COTAs when applicableComplete accurate and timely clinical documentationQualifications:Current Colorado Occupational Therapist (OT) license in good standingGraduate of an accredited Occupational Therapy programStrong clinical assessment, communication, and documentation skillsAbility to work independently in a home health environmentExcellent time-management and organizational skillsValid driver's license, reliable transportation, and auto insuranceHome health experience is preferred but not required.
