Achieve Rehab & Nursing Facility is seeking a compassionate and dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Liberty, New York. This is an outstanding opportunity to work alongside a collaborative interdisciplinary team dedicated to helping residents maximize independence, improve function, and enhance their quality of life.

Responsibilities
Evaluate residents and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy services to short-term rehabilitation and long-term care residents.
Address activities of daily living (ADLs), upper extremity function, cognition, adaptive equipment needs, and functional mobility.
Monitor resident progress and modify treatment plans based on clinical outcomes.
Educate residents, families, and caregivers on therapeutic interventions, safety techniques, and adaptive strategies.
Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes.
Maintain compliance with all federal, state, and facility regulations.
Qualifications
Current New York Occupational Therapist (OT) license or eligibility for licensure.
Graduate of an accredited Occupational Therapy program.
New graduates are encouraged to apply.
Skilled Nursing Facility (SNF) or Long-Term Care experience is a plus, but not required.
Experience with Net Health and PointClickCare (PCC) is preferred.
Strong communication, clinical reasoning, and organizational skills.
Passion for providing compassionate, resident-centered care.
