Home Health Team.
Location: Fayetteville, NC, 28304
Coverage Area: Robeson, Cumberland and Hoke Counties.
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 8 hours/day, 40 hours/week
Productivity Expectation: 30 points/week
On-Call/Weekend Requirement: None
Mileage Reimbursement: Yes, between patient visits (from first patient to last patient)

Traveller Pay package:

Gross Pay: $61.15/Hour (Weekly: $2,446.00)
Take Home pay: $53.95/Hour (Weekly: $2,158.00)
Pay Structure:
Stipend: $1246.00/Week ($31.15/Hour)
W-2 Rate: $1200.00/Week ($30.00/Hour)
Local Pay: $50.00 to $55.00/hour (W-2)

Job Summary:
We are seeking a compassionate and experienced Occupational Therapist (OT) to provide high-quality home health services to patients in Fayetteville, NC, and surrounding coverage areas. The OT will conduct in-home evaluations, develop individualized treatment plans, provide therapeutic interventions, perform reassessments, and complete discharge planning to help patients achieve the highest possible level of independence and functional ability.
Key Responsibilities (KRA):- Conduct in-home OT evaluations, treatments, reassessments, and discharge visits to help patients achieve optimal functional independence and outcomes.
- Develop and implement individualized treatment plans, including ADL training, therapeutic interventions, safety assessments, and recommendations for adaptive equipment or home modifications.
- Complete accurate and timely clinical documentation while maintaining compliance with home health regulations and agency policies.
- Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated, patient-centered care across the assigned coverage areas.
Required Qualifications:- Active North Carolina Occupational Therapist (OT) License and required BLS/CPR/ACLS certification.
- Strong clinical skills with the ability to independently manage patient care in a home health setting.
Preferred Qualifications:- Previous Home Health experience, including OASIS documentation.
- Experience with Homecare Homebase (HCHB) and PointCare for clinical documentation.
