  • Patient Care & Rehabilitation: Provide prescribed occupational therapy services and skilled procedures to minimize physical disabilities and help patients reach their optimum level of independence.
  • Assessment & Care Planning: Conduct initial assessments, develop and modify maintenance plans, perform 30-day reassessments, and collaborate with multidisciplinary home care personnel on care planning.
  • Physician Collaboration: Consult directly with physicians regarding changes in patient condition or treatment plans.

  • Schedule: Monday – Friday (Weekends as needed

Requirements & Qualifications
  • Education & Certification:
    • Graduate of an accredited Occupational Therapy program (CAHEA/AMA/AOTA).
    • Passed or eligible for the AOTA National Certification Exam.
  • Licensure: Current Occupational Therapist license in the state of practice.
  • Experience:
    • Minimum of 1 year of experience in an acute care setting required.
    • 1 year of Home Health experience strongly preferred.
  • Other: Access to reliable transportation.
Compensation: $30.00 - $42.00 per hour
