Position Overview

Bloom Behavior and Consulting Services is seeking a dedicated and compassionate School Nurse for The Bloom Transition Program and the Bloom BCS Clinic in Old Lyme. This role provides nursing support during clinic-based programming, collaborates with clinical and behavioral staff to ensure safe and consistent care, and assists with the health needs enrolled in our program.

Key Responsibilities

• Provide direct nursing care, first aid, and emergency response to clients and students on site, and during community-based activities as needed.
• Administer medications and maintain accurate medication administration records in accordance with district policy and state regulations.
• Develop, update, and monitor Individualized Healthcare Plans (IHPs), and collaborate with IEP teams to support student access and safety.
• Support students with chronic health conditions, mobility needs, seizure disorders, behavioral health needs, and other medically relevant diagnoses.
• Train and support staff in safe health procedures, including universal precautions, emergency protocols, and student-specific medical needs.
• Maintain ongoing communication with families, school staff, and outside providers as appropriate.
• Participate in crisis intervention and safety-team procedures when needed.
• Monitor overall health trends within the program and promote wellness practices.
• Accompany students and staff on community outings and field trips as part of the transition curriculum, as needed.

Required Qualifications

• Current Registered Nurse (RN) license in the State of Connecticut.
• Current CPR/First Aid certification.
• Experience working with adolescents and/or young adults preferred.
• Ability to work collaboratively with multidisciplinary teams, including educators, therapists, job coaches, and administrators.
• Strong communication, organization, and clinical decision-making skills.

Preferred Qualifications

• Experience in special education, transition programming, or working with individuals with developmental disabilities.
• Experience supporting students in community-based vocational or life-skills programs.
• Experience in school setting and/or fast paced enviroment.
• Familiarity with IEP and IHP processes.

Work Schedule

• Position Type: Full Time
• Days: Monday - Friday, following the Bloom Transition Program School Calendar. Opportunity to work summer hours also available.
