Role Overview As a Nurse Practitioner at Duo Health, you’ll be a core member of an interdisciplinary care team, providing comprehensive medical care and chronic disease management to a focused panel of patients living with CKD and other complex conditions. Your work will center on preventive care, chronic disease management, patient education, and care coordination all through a value-based, relationship-centered approach. You’ll work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person. Patients are seen in their homes, virtually, or by phone, giving you the time and context to build meaningful relationships and gain a deeper understanding of each patient’s unique health needs. Work Model & Schedule - Monday–Friday, 8 AM–5 PM (with flexibility based on patient needs)
- No routine weekends/overnights; limited after-hours calls
- 5–6 patient visits/day
- Mix of in-home visits, telehealth, and care coordination
- Daily collaboration with patients and interdisciplinary care team members to coordinate comprehensive, high-quality care
Key Responsibilities - Manage a small panel of patients with CKD and other complex chronic conditions alongside an interdisciplinary care team
- Conduct comprehensive health assessments, histories, physical exams, and ongoing clinical evaluations
- Diagnose, treat, and manage acute and chronic conditions within your scope of practice
- Order, interpret, and review labs and diagnostic testing
- Prescribe and manage medications in accordance with state regulations
- Build individualized care plans addressing each patient's medical, behavioral, and social needs
- Educate patients and caregivers on disease prevention, medication adherence, lifestyle changes, and treatment plans
- Coordinate care across primary care providers, specialists, hospitals, skilled nursing facilities, home health, hospice, and community organizations
- Participate in regular interdisciplinary care team huddles
- Monitor patient progress and adjust care plans to improve outcomes and reduce avoidable hospitalizations
- Document clinical encounters accurately and on time
- Support quality improvement initiatives and evidence-based practice
- Maintain patient confidentiality and comply with company policy and regulatory requirements
- Provide after-hours telephonic support as needed
Qualifications - Active NP/PA license (AZ) + board certification
- Ability to meet Duo Health's clinician credentialing requirements, including a background check
- Reliable transportation, a valid driver's license, and the ability to travel to patient homes
- Strong clinical assessment, communication, and critical-thinking skills
- Comfort working independently while collaborating closely within a team
- Comfort delivering care both in person and via telehealth
- A genuine passion for relationship-centered, value-based care for patients with chronic illness
Preferred - Experience in primary care, internal medicine, geriatrics, home-based care, urgent care, or value-based care
