    Role Overview
    We are seeking a Nurse Practitioner for a flexible part-time opportunity serving patients throughout the Virginia Peninsula. This position provides primary care through a combination of in-home visits and care delivered within assisted living and long-term care communities.
    The provider will typically work 1-2 days per week, coordinating with clinical and scheduling teams to deliver consistent, patient-centered care to adults and seniors across the assigned territory.
    Key Responsibilities
    • Provide comprehensive primary care to patients in home-based and senior living settings.
    • Complete new patient assessments and ongoing follow-up visits.
    • Evaluate, diagnose, and manage acute and chronic health conditions.
    • Create individualized care plans and prescribe medications when clinically appropriate.
    • Order, review, and interpret diagnostic and laboratory testing.
    • Coordinate treatment with specialists and other members of the healthcare team.
    • Provide patients and caregivers with education regarding medications, chronic disease management, prevention, and ongoing care needs.
    • Maintain complete and timely EMR documentation, generally within 24 hours of each patient encounter.
    • Partner with clinical leadership and interdisciplinary team members to support continuity and quality of care.
    Schedule
    • Part-time schedule of approximately 1-2 days per week
    • Flexible scheduling based on provider availability and patient needs
    • Typical patient visit hours: 8:00 AM-5:00 PM
    • No on-call responsibilities
    • Two-week orientation provided
    • Credentialing process may require up to 30 days
    • Optional shadowing opportunities available
    Service Area
    Providers will serve patients throughout the Virginia Peninsula, including:
    • Hampton
    • Newport News
    • York County
    • Poquoson
    Reliable transportation and the ability to travel between patient locations are required.
    Required Qualifications
    • Active Virginia Nurse Practitioner license
    • Current DEA registration
    • Active CAQH profile
    • Minimum of one year of relevant clinical experience in primary care, family medicine, senior care, assisted living, long-term care, or a comparable setting
    • Experience utilizing electronic medical record systems
    • Ability to practice independently while maintaining effective collaboration with an interdisciplinary healthcare team
    • Reliable transportation for local travel throughout the assigned Peninsula territory
    Preferred Qualifications
    • Previous home-based primary care experience
    • Experience treating geriatric, medically complex, or chronically ill patients
    • Clinical background in urgent care, emergency medicine, hospital medicine, or post-acute care
    • Strong communication, organization, and care coordination abilities
