Are you looking for a flexible weekend opportunity providing personalized, patient-centered primary care in the Richmond community? This PRN position allows you to deliver high-quality care to patients in their homes and senior living communities while maintaining a schedule that fits your availability.
Role Overview
This PRN weekend role provides primary care services through a combination of home visits and visits to assisted living and long-term care communities. Providers are scheduled based on weekend patient needs and work closely with the clinical and scheduling teams to ensure continuity of care.
Key Responsibilities
  • Deliver comprehensive primary care during scheduled weekend shifts.
  • Conduct new patient evaluations and follow-up visits.
  • Assess, diagnose, and manage acute and chronic medical conditions.
  • Develop individualized treatment plans and prescribe medications as appropriate.
  • Order and interpret diagnostic testing.
  • Coordinate care with specialists and other healthcare providers.
  • Educate patients and caregivers on disease management and preventive care.
  • Complete accurate EMR documentation within 24 hours of each encounter.
  • Collaborate with the Medical Director and interdisciplinary care team to ensure quality outcomes.
Schedule
  • PRN weekend coverage
  • Flexible scheduling based on availability and patient volume
  • Typical visit hours: 8:00 AM - 5:00 PM
  • No on-call responsibilities
  • Two-week orientation provided
  • Credentialing may take up to 30 days
  • Optional shadowing available
Service Area
  • Richmond, VA (up to a 40-mile service radius)
Minimum Qualifications
  • Active Virginia Nurse Practitioner license
  • Current DEA registration
  • Active CAQH profile
  • At least one year of clinical experience in primary care, family medicine, senior care, assisted living, long-term care, or a related setting
  • Experience with electronic medical record (EMR) systems
  • Ability to work independently while collaborating with an interdisciplinary care team
  • Reliable transportation for travel throughout the assigned service area
Preferred Qualifications
  • Previous experience in home-based primary care
  • Experience caring for geriatric or medically complex patients
  • Background in urgent care, emergency medicine, hospital medicine, or post-acute care
  • Strong communication and care coordination skills
