Compensation: $105,000 - $125,000 Annual Salary, plus RVU bonuses

Job Description: Our Skilled Nursing Facility in Portsmouth, NH is looking for a Nurse Practitioner to join a facility-based team of medical providers caring for patients living in long-term care facilities.

Job Qualifications:

• Current, unrestricted NP license in the state of New Hampshire
• Board certification
• DEA licensure
• Clinical background in adult, family or geriatric specialties
• Experience working in a long-term care setting

Responsibilities include but are not limited to:

• Provide primary and preventative care for long-term care patients residing in skilled nursing facilities
• Manage residents with an acute change in condition
• Advanced care planning and chronic care management
• Order, review and follow up on treatments and diagnostic testing
• Communicate regularly with nursing home staff and collaborate with the primary care physician
