Your Role
- Deliver high-quality care to seniors in their homes, group homes, and independent living settings
- Diagnose and manage complex chronic conditions
- Collaborate with a multidisciplinary team (physicians, pharmacists, nurses, MAs, and more)
- Educate patients and families, including discussions on goals of care and end-of-life planning
- Use critical thinking and evidence-based practices
- Document thoroughly using modern EMR systems and technology tools
- Represent At Home Harmony with professionalism and empathy
Qualifications
- Minimum 2 years as an NP/PA in geriatrics, internal medicine, family practice, or palliative care, preferred
- Tech-savvy: Familiarity with EMRs (Athena), Microsoft Office, mobile tools, and AI features
- Experience in home-based care preferred, but not required
- Graduate from an accredited NP or PA program
- Active, unrestricted license and national certification
- BLS, DEA, and prescriptive authority
- Valid driver's license and reliable transportation
- Ability to lift up to 50 lbs and navigate stairs

- Job Type: Full-Time or Part-Time - Up to you!
- Compensation: Competitive salary with bonus potential
- Schedule: Monday–Friday, flexible between 8 AM – 5 PM
