Nurse Practitioner/Physicians Assistant- LTC

Pride Health is hiring a Nurse Practitioner/Physician Assistant - LTC to support our client's healthcare facility in Pennsylvania. 

Job Details:

• Schedule: Monday-Friday | 8:00 AM-5:00 PM | No On-Call/Weekends/Holidays
• Location: Clearfield and Centre Counties, PA; Travel between 5 facilities during shift. J
• Job Type: Contract
• Pay Range: $69-$72

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Conduct comprehensive patient assessments.
• Manage acute and chronic medical and behavioral conditions.
• Ensure accurate and complete documentation, including ICD-10 conditions. 
• Communicate and collaborate with interdisciplinary care teams.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.

Requirements:

• License(s): Active and unrestricted NP or PA license in the state of residence. 
• Certification(s): Active national board certification; NP certification through AANP or ANCC in an approved specialty, or PA certification through NCCPA.
• Education: NP must be a graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program. PA must be a graduate of an accredited Physician Assistant degree program.
• Years of experience: 1+ year of clinical experience preferred; long-term care experience preferred. 
• Skills/Specialties/Technology: Experience with complex medical, behavioral, social, or functional needs; understanding of geriatrics, chronic illness, advanced illness, and end-of-life discussions.
