Nurse Practitioner/Physician Associate

Who You Are:

Amoskeag Health is seeking a full-time Nurse Practitioner or Physician Associate who delivers compassionate, comprehensive outpatient care across the lifespan. ~ This position may be filled by either a Nurse Practitioner or a Physician Associate. ~

What You'll Do:

• Provide comprehensive, family-centered outpatient care across the lifespan, including routine exams, diagnosis and treatment, patient education, and follow-up care in alignment with Amoskeag Health's policies and clinical protocols.
• Make appropriate referrals to internal departments and external community partners (e.g., primary care, specialists, behavioral health, nutrition, social services, substance use treatment).
• Deliver acute care services, including same-day visits as needed, in coordination with nursing staff triage and scheduling.
• Conduct timely and appropriate follow-up through phone or written communication, including notification of abnormal test results, in collaboration with nursing staff.
• Assist in developing clinical policies and protocols related to care across all ages and stages of life.
• Provide individual and group health education to patients and families.
• Consult with nurses providing triage for walk-ins and phone calls and direct referrals to specialty services when appropriate.
• Maintain clear, accurate, and timely documentation in the electronic medical record, ensuring information is easy to understand for all care team members.
• Communicate clearly with patients, families, and internal staff through both written and verbal channels.
• Provide outreach and consultation to community organizations when applicable.
• Participate in case conferences and care-management meetings to support integrated patient care planning.
• Attend required clinical and administrative meetings; review and sign meeting minutes when unable to attend.
• Support requirements tied to organizational funding sources, including assistance with grant development, implementation, and reporting as needed.
• Attend mandatory safety and infection-control trainings as assigned.
• Collaborate with staff to develop and maintain Nurse Practitioner-related policies and procedures.
• Participate in quality-improvement initiatives, projects, and additional clinical or administrative work that supports high-quality patient care.
• Contribute to data collection, reporting, and future program planning within your area of expertise.
• Maintain professional licensure and certification through ongoing education and re-certification requirements mandated by the State of New Hampshire.
• Prescribe medications appropriately based on patient needs and clinical protocols.
• Provide culturally competent care and maintain awareness of age-specific needs of the patients served.

Qualifications:

• Master's degree and current New Hampshire Nurse Practitioner license required.
• One to five years' experience as a Nurse Practitioner in a multi-disciplinary team providing primary health care to medically indigent families preferred.
• American Heart Association (AHA) BLS Certification required - candidates without current certification will be required to complete an Amoskeag Health-provided AHA BLS class upon hire (must be maintained).

What We Offer:

• Compensation for this role is $55- $67.50/hour (salaried)
• This position is 40 hours per week, onsite at Amoskeag Health with 34 hours patient facing and 6 hours of admin time (flexible).
