Overview

Correctional Healthcare Partners (CHP) is seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care within a correctional setting.

This role offers the opportunity to practice at the top of your license, providing comprehensive medical care to an underserved population while working collaboratively within a multidisciplinary team. The APP will play a key role in improving patient outcomes, ensuring continuity of care, and supporting CHP's mission of delivering accessible, quality healthcare.


Key Responsibilities

  • Perform comprehensive health assessments, including physical exams, medical histories, and diagnostic evaluations
  • Diagnose and manage acute and chronic medical conditions
  • Develop and implement individualized treatment plans
  • Prescribe medications and treatments in accordance with state regulations and CHP protocols
  • Order, interpret, and evaluate diagnostic tests and lab results
  • Provide patient education on disease prevention, health maintenance, and treatment plans
  • Respond to medical emergencies and provide urgent care as needed
  • Collaborate with physicians, nursing staff, mental health clinicians, and custody staff to ensure coordinated care
  • Maintain accurate, timely, and compliant documentation in the electronic health record (EHR)
  • Participate in quality improvement initiatives and adherence to clinical protocols
  • Support discharge planning and continuity of care upon release or transfer

Qualifications

  • For Nurse Practitioners (NP):
    • Active and unrestricted NP license in the applicable state
    • Board certification (FNP, AGNP, or related)
    • Current DEA license (or ability to obtain prior to start)
  • For Physician Assistants (PA):
    • Active and unrestricted PA license in the applicable state
    • Graduate of an accredited PA program
    • NCCPA certification required
    • Current DEA license (or ability to obtain prior to start)
  • Minimum 2–3 years of APP experience in primary care, urgent care, emergency medicine, or a related setting
  • Experience in correctional healthcare or working with underserved populations preferred but not required
  • Strong clinical assessment and decision-making skills
  • Ability to work independently within scope while collaborating with supervising physicians and interdisciplinary teams
  • Excellent communication and interpersonal skills

Pay: $95-$100/hr

Schedule: 12pm to 10pm on Thursdays, Fridays, Saturdays, and Sundays
