The Opportunity
This Nurse Practitioner/Physician Assistant role is an opportunity to become a key clinical partner to our PM&R physicians in skilled nursing facilities. You will focus on improving patient outcomes through thoughtful rehab planning and close coordination with an interdisciplinary care team.

What You'll Do
- Partner on-site with Board-Certified PM&R physicians to deliver comprehensive rehab and care to patients in skilled nursing facilities.
- Collaborate daily with therapy departments, nurses, social workers, and families to develop and adjust patient-centered rehabilitation plans.
- Conduct assessments, manage medications and pain regimens, and monitor functional progress with a focus on maximizing patient independence and comfort.
- Communicate regularly with patients' primary care and specialty providers to ensure coordinated, high-quality care and smooth transitions.
- Complete timely, accurate documentation and charting, including work that can be done from home, to support clinical quality and regulatory requirements.
What Success Looks Like
- Within the first 6–12 months, you are a trusted clinical presence in your assigned facilities, known for clear communication and reliable follow-through with patients, families, and staff.
- Patients under your care demonstrate improved functional outcomes and pain control, contributing to higher satisfaction scores and quality indicators.
- Readmission rates and avoidable complications are reduced through proactive management, early intervention, and strong coordination of care.
- Documentation is consistently timely, thorough, and compliant, supporting smooth care transitions and positive relationships with partner facilities.
What We're Looking For
- Licensed and certified Nurse Practitioner or Physician Assistant with active eligibility to practice and a current license in Michigan.
- Commitment to detail-oriented, outcome-focused care, with a genuine interest in rehabilitation medicine and working with senior populations.
- Collaborative, team-oriented approach with strong communication skills and the ability to work effectively with interdisciplinary teams and families.
- Reliable transportation to travel to skilled nursing facilities and the ability to manage your time and schedule responsibly in a flexible work environment.
