    The Opportunity

    We are seeking a Nurse Practitioner or Physician Assistant licensed in Tennessee to partner with our PM&R physician in skilled nursing facilities. This part-time, in-person role is ideal for an advanced practice provider who is passionate about rehabilitation, enjoys building relationships with patients and staff, and values a stable, team-oriented environment.

    You will have a flexible schedule with no on-call responsibilities, the ability to complete documentation from home, and the chance to make a measurable difference in patient function, satisfaction, and rehospitalization rates.


    What You'll Do

    • Partner with a Board-Certified PM&R physician to manage patients in sub-acute rehab and skilled nursing facilities.
    • Provide on-site pain management and rehabilitation-focused care to improve patient functionality and comfort.
    • Collaborate with facility therapy departments to align medical and rehab plans of care.
    • Coordinate care and communicate regularly with primary care providers, therapists, social workers, nurses, and families to ensure cohesive treatment plans.
    • Complete timely, accurate charting and documentation, with flexibility to perform documentation tasks from home.

    What Success Looks Like

    • Within the first 6–12 months, you are a trusted clinical partner to facility staff, reliably managing a panel of rehab patients with minimal supervision.
    • Patient functionality and satisfaction scores improve through consistent, thoughtful pain management and rehab-focused interventions.
    • Readmission rates related to pain and functional decline are reduced through proactive, on-site care and early intervention.
    • Communication with families and interdisciplinary teams is timely and clear, resulting in coordinated care plans and smooth transitions.

    What We're Looking For

    • Licensed and certified Nurse Practitioner or Physician Assistant with active Tennessee licensure.
    • Strong interest in Physical Medicine, Rehabilitation, or geriatric care within skilled nursing or sub-acute rehab settings.
    • Detail-oriented, outcome-focused clinician who values teamwork, stability, and people-centered care.
    • Reliable transportation and the ability to work in-person at skilled nursing facilities while managing documentation efficiently.
