Key Responsibilities:

• Deliver comprehensive primary care services, including preventive screenings, diagnosis, and treatment.
• Integrate SDOH screenings and interventions into patient assessments and care plans.
• Participate in continuous quality improvement initiatives to meet and exceed PCMH quality metrics.

What Youll Bring Active NP license and DEA certification for Louisiana. Experience in primary care, acute care, or urgent care preferred. Strong critical thinking, communication, and independent decision-making skills. Passion for flexible, patient-centered care in metro and under-served areas.
