Key Responsibilities:
- Deliver comprehensive primary care services, including preventive screenings, diagnosis, and treatment.
- Integrate SDOH screenings and interventions into patient assessments and care plans.
- Participate in continuous quality improvement initiatives to meet and exceed PCMH quality metrics.
What You’ll Bring
- Active NP license and DEA certification for Louisiana.
- Experience in primary care, acute care, or urgent care preferred.
- Strong critical thinking, communication, and independent decision-making skills.
- Passion for flexible, patient-centered care in metro and under-served areas.
