Nurse Practitioner – Outpatient Vascular Care
Location: Queens or Brooklyn| Schedule: Day Shift |
 
Description
Step into a mission-driven role where your clinical judgment directly saves limbs and changes lives. As the lead Nurse Practitioner for a growing company, you will:
 
  • Partner with interventional specialists to evaluate and treat patients with peripheral artery disease and other complex vascular conditions.
  • Design and execute evidence-based care plans that restore blood flow and prevent lower-extremity amputations.
  • Perform comprehensive exams, order/interpret diagnostics, and prescribe therapies, documenting each encounter in a voice-recognition–enabled EMR.
  • Provide continuity of care across sites, including scheduled visits to partner nursing homes .
  • Educate and empower patients and families, applying critical-thinking skills to every interaction.
  • Contribute to quality-improvement and case-review initiatives that drive superior outcomes.
 
Requirements
  • Current NY State licensure and board certification as a Nurse Practitioner.
  • Minimum 2 years’ experience in a high-acuity environment such as vascular surgery, operating room,  interventional radiology, critical care, trauma, or cardiology.
  • Proven ability to craft comprehensive visit notes and autonomous care plans.
  • Exceptional communication, patient-education, and clinical-decision skills.
  • Wound-care and PAD experience a plus (will train the right candidate).
  • Valid driver’s license.
