If you havent considered in-home primary care, heres why you should.
At HarmonyCares, our model is built around value-based care quality over quantity. - Practice within a value-based care model focused on quality and outcomes rather than visit volume
- Patient visits are geographically clustered to help minimize unnecessary travel
- Technology and clinical support designed to give providers more time to focus on patient care
- Limited after-hours telephonic call shared across the provider team
