Description

Loyola Medicine, a member of Trinity Health, is a nationally ranked academic, quaternary care health system serving Chicago's western suburbs. Anchored by Loyola University Medical Center, the system includes three hospitals: Loyola University Medical Center, Gottlieb Memorial Hospital, and MacNeal Hospital.

We are seeking an Advanced Practice Provider to join our Advanced Heart Failure Program, supporting patients with advanced heart failure, Ventricular Assist Devices, and heart transplants. This role combines inpatient and outpatient responsibilities and offers the opportunity to contribute to a growing program dedicated to expanding access to specialized cardiovascular care across the health system.

Position Overview

  • Provide comprehensive inpatient care for advanced heart failure, VAD, and transplant patients, including assessment, treatment planning, admissions, transfers, and multidisciplinary care coordination.
  • Partner with physicians and interdisciplinary teams to optimize patient outcomes through daily rounds, clinical documentation, order management, and transplant evaluations.
  • Coordinate discharges and transitions of care while ensuring seamless continuity between care teams.
  • Support the evaluation and management of transplant candidates and patients awaiting advanced therapies.
  • Expand access to specialty care through growing ambulatory and outpatient services.
  • Contribute to initiatives focused on improving care coordination, enhancing patient access, and reducing hospital readmissions.

Qualifications

  • CPR - Basic Life Support
  • ACLS - Advanced Cardiovascular Life Support
  • Nursing - Current Registered Nurse License - State of Illinois (RN)
  • Nursing - Nursing Practitioner License - State of Illinois (APN)
  • Physician Assistant License - State of Illinois (PA)

Compensation & Benefits

  • Pay Range: $109,304 - $180,336 per year
    Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
