Nurse Practitioner or Physician Assistant for our Skilled Nursing Facility (SNF)

Compensation: $115,000 - $152,000 Annual Salary

This is a full time, in-person position. The provider rounds on skilled nursing and long-term care residents, manages acute changes in condition, chronic disease and advanced care planning, and coordinates with the interdisciplinary team and collaborating physician.

Qualifications:

• Active Massachusetts NP (FNP or AGNP) or PA license in good standing
• DEA licensure
• Experience in a skilled nursing or long-term care setting preferred
• Ability to round independently and manage a medically complex geriatric panel

Responsibilities:

• Provide primary and preventative care to residents in a skilled nursing facility
• Manage residents with an acute change in condition
• Advanced care planning and chronic care management
• Order, review and follow up on treatments and diagnostic testing
• Communicate with facility nursing staff and collaborate with the primary care physician
