    Position Overview
    An established employer-sponsored healthcare clinic in Fort Wayne, Indiana is seeking an experienced Nurse Practitioner to serve as the primary clinical provider in a small, patient-focused outpatient setting. This role is ideal for a provider who is comfortable practicing independently, managing a broad range of primary care needs, and supporting wellness and preventive health initiatives.
    The clinic team consists of one provider, one medical assistant, and one health coach, creating a collaborative environment focused on personalized patient care.
    Schedule
    • Monday: 7:30 AM - 4:30 PM
    • Tuesday - Thursday: 8:00 AM - 5:00 PM
    • Friday: 7:30 AM - 4:30 PM
    • No travel required
    Key Responsibilities
    • Conduct pre-employment physicals and annual wellness examinations
    • Assess, diagnose, and manage acute and chronic medical conditions using evidence-based practices
    • Develop and implement individualized treatment plans in accordance with supervising physician protocols
    • Evaluate and treat workplace injuries and common illnesses
    • Deliver preventive health services, screenings, and wellness initiatives
    • Provide patient education, health coaching, and lifestyle counseling
    • Support patients with weight management goals through nutrition, exercise, behavioral interventions, and medication management, including GLP-1 therapies when appropriate
    • Order, interpret, and review diagnostic testing
    • Monitor patient outcomes and adjust treatment plans as needed
    • Coordinate care and make referrals to specialists or other healthcare professionals when appropriate
    • Maintain accurate and timely electronic medical records documentation
    • Collaborate with clinic staff and leadership to ensure high-quality patient care
    • Assist with clinic operations and other duties as needed
    Required Qualifications
    • Board Certified Nurse Practitioner with an active state license
    • Minimum of 1 year of experience in Family Medicine, Internal Medicine, Urgent Care, or a related primary care setting
    • Ability to practice confidently as the sole provider within a clinic environment
    • Strong clinical assessment and decision-making skills
    • BLS Certification
    • Excellent communication and patient engagement abilities
    • Commitment to patient-centered care and preventive medicine
    Preferred Qualifications
    • DEA License
    • Experience in employer-sponsored, occupational health, or wellness clinic settings
    • Experience supporting patients with weight management programs
    • Familiarity with obesity treatment medications, including GLP-1 therapies
    • Experience collaborating with multidisciplinary healthcare teams
