Compensation:
Pay Range: $140,000-$170,000 Per Year

About the Role:
• Client is seeking a Bilingual English/Spanish Nurse Practitioner (NP/FNP) or Physician Assistant (PA) to join its growing Primary Care team.
• This is an excellent opportunity for a provider who is passionate about delivering high-quality, patient-centered care in a collaborative environment.
• You'll work alongside physicians, specialists, nurses, health coaches, and support staff to improve patient outcomes while building long-term relationships with a diverse patient population.

What You'll Do:
• Provide comprehensive primary care services for adult patients.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform physical examinations and order, interpret, and review diagnostic tests.
• Develop individualized treatment plans and educate patients on preventive care and chronic disease management.
• Coordinate care with physicians, specialists, and interdisciplinary team members.
• Document patient encounters accurately using the electronic health record (EHR).
• Participate in quality improvement initiatives and population health programs.
• Support team-based care within a Patient-Centered Medical Home (PCMH) model.
• Maintain compliance with HIPAA and organizational policies.

Qualifications Required:
• Master's Degree from an accredited Nurse Practitioner or Physician Assistant program.
• Active New York State NP or PA license.
• Board Certified Nurse Practitioner (FNP, AGNP, ANP, ENP) or Physician Assistant.
• Bilingual in English and Spanish (conversational and/or medical fluency required).
• Background in Family Medicine, Internal Medicine, or Primary Care.
• Strong communication, organizational, and interpersonal skills.
• Experience using Electronic Health Records (EHR).

Qualifications Preferred:
• Prior experience in an Article 28 ambulatory care center, community health setting, or PCMH practice.
• Experience working with culturally diverse patient populations.
• Passion for preventive medicine, quality improvement, and team-based care.
