We’re hiring a Nurse Practitioner who wants predictable weekday hours, meaningful relationships with senior patients, and the opportunity to earn $143,000+ through base salary and incentives.

You’ll step into an outpatient primary care role focused on seniors with complex chronic conditions. You’ll have your own care team and strong leadership support while practicing within a value-based care model.

• Approximately 10–12 patients per day
• Maximum panel of 650 patients
• Call frequency based on the provider team size

What makes you a great fit

• You are a Nurse Practitioner with 2+ years of outpatient primary care experience
• You’re comfortable caring for seniors with multiple complex conditions
• You can perform suturing, joint injections, skin biopsies, and basic wound care
• You hold an active, unrestricted license and current DEA registration
• You value patient-centered, team-based care
• Geriatric experience is a plus, but not required

Compensation & Benefits

• Base salary: $130,000
• Up to $13,000 annual bonus
