Practice Highlights / Qualifications
- Full-time post-acute care position
- Round in skilled nursing and long-term care facilities
- Conduct patient assessments, develop treatment plans, and manage ongoing care

- High level of autonomy with strong operational support
- Connecticut NP or PA license required (or ability to obtain)
- DEA required
- Experience in post-acute, primary care, hospital medicine, emergency medicine, or geriatrics preferred
