Seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a full-time Post-Acute Care opportunity in Greensboro, North Carolina. Practice Highlights / Qualifications

• Full-time schedule (5 days per week)
• Round in skilled nursing and post-acute care facilities
• Perform comprehensive patient assessments and physical exams
• Develop and manage individualized care plans
• Coordinate care with physicians, nursing staff, and interdisciplinary teams
• Educate patients and families on treatment plans and preventive care
• North Carolina NP or PA license and DEA required
• Experience in post-acute, acute care, emergency medicine, or clinic settings preferred but not required
