    Nurse Practitioner (NP) – Complex Care

    Location: Delaware
    Schedule: Full-Time | Telehealth & Remote Care

    Our client is seeking an experienced Nurse Practitioner to join a collaborative clinical team providing proactive, technology-enabled care to medically complex patients throughout Delaware.

    This is an opportunity to move beyond the traditional high-volume clinic and focus on care coordination, prevention, and improved patient outcomes—helping vulnerable patients remain safely at home.

    What You'll Do

    • Provide clinical care and ongoing management through telehealth and remote technologies
    • Manage medically complex adult and geriatric patients
    • Develop individualized care plans with physicians and an interdisciplinary team
    • Coordinate care with patients, families, caregivers, specialists, and providers
    • Support hospital transitions and help prevent avoidable ED visits and hospitalizations
    • Participate in advance care planning and goals-of-care discussions
    • Support quality initiatives, HCC coding, risk adjustment, and quality-gap closure

    Qualifications

    • Active NP license in Delaware or a bordering state; Delaware licensure support provided
    • FNP or Adult/Adult-Gerontology NP certification
    • 3–5+ years of NP experience
    • Experience with medically complex adult/geriatric populations
    • Telehealth or remote-care experience
    • Knowledge of HCC coding/risk adjustment and quality measures
    • Strong communication and clinical decision-making skills
