    Pay Range: $50.00 - $65.00/hour

    Job Description

    We are seeking a compassionate Hospice Nurse Practitioner (NP) to provide high-quality hospice and palliative care to patients with serious and terminal illnesses. Working collaboratively with the Hospice Medical Director and interdisciplinary team, you will conduct patient assessments, manage symptoms, perform hospice eligibility evaluations, and develop individualized plans of care that support patients and families during end-of-life care.

    Key Responsibilities

    • Conduct comprehensive patient assessments, including history and physical exams.

    • Perform required hospice face-to-face visits and evaluate eligibility for hospice certification and recertification.

    • Develop and manage individualized treatment plans.

    • Prescribe medications and treatments in accordance with state regulations.

    • Collaborate with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care.

    • Manage pain and symptom control to improve patient comfort and quality of life.

    • Educate patients, families, and caregivers on disease progression, symptom management, and end-of-life care.

    • Complete timely and accurate clinical documentation.

    • Ensure compliance with Medicare, Medicaid, HIPAA, and hospice regulatory requirements.

    Qualifications

    • Active and unrestricted Nurse Practitioner (NP) license.

    • Master's degree in Nursing from an accredited program.

    • Current CPR certification.

    • Minimum 1 year of Nurse Practitioner experience.

    • Minimum 1 year of hospice or palliative care experience preferred.

    • Strong clinical assessment, communication, and care coordination skills.

    • Valid driver's license, reliable transportation, and auto insurance.
