Imagine360 is seeking a Contract Nurse Practitioner to join the team! miCare delivers on-site primary care clinics to remove both access and cost barriers for enrolled health plan members, offering greater opportunity for education and engagement. This Nurse Practitioner position is responsible for delivering office based primary care for males and females of all ages; includes chronic disease management with an emphasis on wellness along with acute episodic care. Position Location: Clovis, CAResponsibilities include but are not limited to:General DutiesUtilizes knowledge and skills to care for patients under the conformance of current and recognized techniques, procedures and standards. Includes but not limited to:physical examinationsorders, interprets and evaluates diagnostic tests to assess patients clinical problems and health care needsrecords findings and formulates plan and prognosis prescribes necessary prescriptions and other forms of treatmentrefers appropriately to other fields of specializationperforms effective communication with miCare team and patient to ensure coordination of care.Customer ServiceActs as a role model in demonstrating the core values in customer service delivery. Provides timely and thorough follow up with, internal and external customers.Appropriately escalates difficult issues up the chain of command.Quality AssuranceOffers feedback and assistance in improving customer satisfaction.Recognizes and alerts appropriate supervisor of trends within their scope of responsibility that fall outside of quality parameters.Performs self quality monitoring in order to develop and execute plans to meet established goals.Collaborates with others and cross-departmentally to improve or streamline procedures.Provides feedback and develops new or improves current internal processes to optimize qualityTrains new employees as assignedRequired Experience/Education:Degree from accredited schoolCurrent unencumbered license to practice in the state of the Health CenterMinimum of three years clinical experienceCurrent BLSWorking knowledge of computers and software including but not limited to Microsoft Office productsSkills and Abilities:Proficient typing skillsDemonstrated organizational skills, problem-solving, analytical skills and detail oriented Strong written and oral communication skillsDemonstrated ability to work independently, prioritize workloads multi-task and manage priorities in order to meet deadlinesAbility to maintain the confidentiality of protected health information in compliance with HIPAA regulationsLicense and Certifications: Must maintain an active, current, Nurse Practitioner license/certification in good standing, including all continuing education requirements as mandated by the California Board of Registered Nursing. Must maintain current Basic Life Support (BLS) certification.
