A growing palliative care organization is seeking an experienced Nurse Practitioner to provide high-quality patient care while helping lead and mentor a team of advanced practice providers across New Jersey.
This hybrid role combines direct patient care with clinical leadership and offers the opportunity to make a meaningful impact on patients living with serious illnesses and complex chronic conditions.

• Provide palliative care assessments and follow-up visits• Manage patients with serious illness and complex chronic conditions• Facilitate goals-of-care and advance care planning discussions• Collaborate with physicians, specialists, hospitals, and community providers• Mentor and support Nurse Practitioners and clinical staff• Participate in quality improvement and operational initiatives• Assist with onboarding, training, and team development
• Active New Jersey Nurse Practitioner license• National board certification and DEA registration• Minimum 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management• Strong communication and leadership skills• Experience working within interdisciplinary care teams
• Leadership or supervisory experience• Hospice & Palliative Care certification (ACHPN)• Experience with home-based care, telehealth, or value-based care models• Bilingual skills are a plus
