    Position Summary

    Seeking a passionate and experienced Full Time Nurse Practitioner (NP) that specializes in Family Medicine! The ideal candidate will effectively collaborate with primary care physicians, provide primary care to patients with complex chronic and acute health conditions. The NP will conduct a medical intake for each patient, perform physical examinations, formulate diagnosis's, develop treatment plans for patients, maintain accurate records and provide health education to patients on how to manage their diagnosis/illness.

    The ideal candidate for this role, Full Time Nurse Practitioner - Family Medicine, will be passionate about providing care and educating the patients in this community!  You will be given the opportunity to make an impact and change the lives of countless people from all different walks of life!  The core responsibilities for this role will be the following:

    • Delivery of a variety of healthcare services, including preventative, chronic and acute care for a wide variety of patients (infant to geriatric)
    • Perform diagnosis and treatment of illnesses, carry out health-risk assessments, screening tests, and counseling to individuals on the Observation Unit
    • Ensure participation in continuing education and training to keep abreast of current industry developments 
    • Provide quality care to patients by following the most current evidence-based guidelines and procedure of practice
    • Maintain accurate patient records and participate in quality assurance audits 
    • Provides medical and mental health education services to patients, their families, and care-takers
    • Obtains detailed medical history and perform physical examination on assigned patients
    • Diagnoses and prescribes treatments, both pharmacological and nonpharmacological 
    • Develops and implements patient management plans, records progress notes, and assists in the provision of continuity of care
    • Counsels and instructs patients regarding compliance with prescribed therapeutic regimens and health maintenance 
    • Collaborate with supervising physician and other members of the healthcare team in care coordination / clinical meetings
