    Position Summary

    Seeking a compassionate, detail-oriented, and patient-focused Primary Care Nurse Practitioner to provide comprehensive healthcare services to patients across the lifespan. The NP will work collaboratively with physicians, nurses, and interdisciplinary staff to deliver high-quality primary and preventive care in an outpatient setting.

    Responsibilities

    • Conduct comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations
    • Diagnose and treat acute and chronic medical conditions
    • Develop and implement individualized treatment and care plans
    • Prescribe medications and therapies in accordance with state regulations and practice guidelines
    • Order, interpret, and review laboratory tests, imaging, and other diagnostic studies
    • Provide preventive care services, including annual physicals, screenings, vaccinations, and health education
    • Manage chronic diseases such as diabetes, hypertension, asthma, and cardiovascular conditions
    • Collaborate with physicians, specialists, and healthcare team members regarding patient care plans
    • Maintain accurate and timely electronic medical records (EMR) documentation
    • Educate patients and families on disease prevention, medication management, and healthy lifestyle choices
    • Participate in quality improvement initiatives and compliance activities
    • Ensure adherence to HIPAA regulations and organizational policies

    Qualifications

    • Current New York State Nurse Practitioner license
    • Board Certification
    • DEA registration
    • Experience with EMR systems preferred
    • Strong clinical judgment, communication, and interpersonal skills

    Schedule & Work Environment

    • Full-time
    • Outpatient primary care setting

    Benefits

    • Competitive salary $150,000-$175,000
    • Full benefits package
