Responsibilities Include:
- Conducting virtual patient assessments and developing treatment plans
- Providing remote clinical services to assisted living residents
- Documenting all care accurately and thoroughly
- Collaborating with the clinical team to support patient outcomes
Ideal Qualifications:
- Must be Medicaid-credentialed
- Strong clinical assessment and communication skills
- Detail-oriented and comfortable with virtual care platforms
- Prior geriatric or assisted living experience is a plus
